Privacy Statement

 It is our responsibility at Chimo to protect the personal information shared with us by our clients.  We are committed to ensuring that we are following the established legislative requirements to safeguard your information under the Personal Health Information Protection Act (PHIPA) and following the privacy standards as set out by the Information and Privacy Commissioner of Ontario (IPC).   All of our staff sign confidentiality agreements at the time of hire.  

 

The information you share is confidential to Chimo and not shared with others without your consent. You may withdraw or withhold your consent at any time.  There are some situations in which we may have to share information without your consent to ensure the safety of you and/or others, or when we are required to comply with the law, this is referred to as our duty to report.

 

Your personal information is never destroyed; it is always kept in electronic and/or hard copy format.  Hard copy information is securely stored and locked so that only authorized personnel have access.  Files taken offsite are kept locked up at all times when not on the person directly. All electronic file data is restricted to authorized personnel only, and data that is saved to backups or online is fully encrypted for your protection. We continually update and improve our security.  We have other safeguards in place to ensure access to your personal information is secure.  You may request to see our privacy policies at any time. 

 

Should there be a privacy breach in which your information is lost, stolen, used, or disclosed without authorization, we are obligated to notify you immediately as well as report any such incidents to the Privacy Commissioner. 

 

You may request access to your information at any time by contacting the Privacy Officer at Chimo.  You will be asked to verify your identity before any information is shared or released.  There are some extenuating situations in which access to information may be denied; as per legislation, a reason will be provided to you should this be the case.  Should you wish to have any of your personal information corrected, you may provide a written request detailing the correction and sending to the Privacy Officer, who will ensure such information is added to your file.  

 

The Privacy Officer for Chimo is the Director of Administration.  Any concerns about the privacy of your information should be addressed with your worker and/or their supervisor; however, if you are not satisfied with the response or would rather correspond with the Privacy Officer, you may do so by calling (705) 324-3300 ext. 227 or by sending an email to teresa@chimoyouth.ca
